Queen Mary Concern for Student Form
Who is this information for
Queen Mary staff who have serious concerns about the welfare, wellbeing or safety of a Queen Mary Student.
Immediate risk of harm
Do not use this form in an emergency. If a student is at immediate risk of serious harm to themselves or others, call 999 for the emergency services.
If you are on campus and have called 999, please also contact Queen Mary Security on 020 7882 3333 or internal extension 3333 so they can meet and direct the emergency services.
In situations where you deem it is safe for the student to travel to A+E without emergency services (where they appear able to understand the situation, make decisions, and keep themselves safe during the journey) they should be supported to travel with another person.
A trusted friend, flatmate, colleague or other appropriate person should be advised to accompany the student, whether they are walking, using public transport or taking a taxi. They should remain with the student until they have arrived at A+E, registered, and been taken through for assessment by hospital staff.
For students on or near Queen Mary’s Mile End or Whitechapel campus, the nearest A+E is usually The Royal London Hospital. For students at other Queen Mary campuses or locations, staff should check the nearest appropriate A+E department before advising the student to travel.

When to use this form
Queen Mary staff should use this form when they have a serious concern about a student’s health, welfare, safety, mental health or wellbeing. Please provide as much relevant information as possible.
If your concern relates to a student who is under 18, or an adult at risk, please use the Queen Mary Safeguarding process instead. If you are unsure, please still submit the form.

What happens next
The form will be reviewed by the Advice and Counselling Service (ACS), who will assess the concern and, where appropriate, contact the student to offer support.
ACS works confidentially. Information about a student’s contact with the service will not normally be shared without their consent. More information is available on the ACS data protection and confidentiality page.
Telling the student
Where it is safe and appropriate to do so, please inform the student that you are submitting this form and, ideally, seek their consent to a referral to the Advice and Counselling Service. Students are often more likely to engage with support when they know a referral has been made.
If you cannot gain consent, or if telling the student may increase risk, please still submit the form and explain:
- Whether the student knows about the referral
- Whether they have consented
- Why it was not possible or appropriate to discuss it with them.

If you are unsure what to do, you can request an anonymous consultation through this form. Please indicate whether your concern relates to:
- Mental health or emotional wellbeing
- Sexual assault or harassment
- Welfare issues, including domestic or familial abuse, including so-called 'honour' based abuse, and/or risk of forced marriage, homelessness, estrangement or urgent financial difficulty
- Immigration advice
- Other (please specify)
A relevant ACS colleague will contact you to arrange a suitable time to discuss the concern.

Report and Support
Students who have experienced bullying and harassment, sexual violence, hate incidents or gender-based violence, can report it to Queen Mary using Report + Support, but you can use this form to flag any serious concerns that relate to the student.
