Medical certificates for Extenuating Circumstances (Suttons Wharf Health Centre)
If you are registered with Suttons Wharf Health Centre (Student Health Service) and need a medical certificate to support an Extenuating Circumstances (EC) claim, please follow the process below.
Medical certificates are based on a clinical consultation and are normally completed within 7 days of submitting your request.
How to request a medical certificate
Step 1 – Request a GP consultation
Submit an online consultation request via Suttons Wharf Health Centre to arrange a GP review. This may be:
- face-to-face
- telephone
- e-consult
Step 2 – Discuss your request
During your consultation, explain your circumstances and why you require a medical certificate.
Step 3 – Complete the medical evidence form
After your consultation, complete the medical evidence form (see below). You will be asked to provide:
- a description of your condition or issue
- when it started and how long it lasted
- when you were seen by the GP
- how it affected your studies
Submit the completed form to reception in the Student Health Service or Suttons Wharf Health Centre and pay the applicable fee.
Step 4 – Certificate completion
The form will be reviewed and signed by a GP. The maximum turnaround time is 7 days from submission.
Step 5 – Collection
The Health Centre will contact you when your form is ready for collection.
Important information
- You should seek medical advice as soon as possible if you are unwell.
- Medical certificates can only be issued where a Student Health Service/Suttons Wharf Health Centre GP has assessed your condition.
- Certificates are based on information recorded in your medical notes at the Student Health Service.
- A fee applies for medical certificates (currently £20).
